Please download, complete and email back to me the below pdf forms found at the bottom of this page at least three days prior to your first session. If you are a couple, you will need to each fill out a set. Please print out pages one and two and bring them with you to your first session.
Please call or email me for my fees. One or two day intensives are also available upon request. I offer Skype and Face Time if you live in the State of Wisconsin. I offer a sliding scale if you qualify. I accept checks, credit cards and cash in the exact amount due. I also offer a complimentary 20 minute session to get a feel for how we'd work together.
I have a fee for service business. I am not paneled with any insurance companies, however, I can be added as an "Out of Network Provider." I will provide you with a receipt that you submit to your insurance company for reimbursement. It is up to you to request my services being added as an AASECT Certified Sex Therapist or a Certified Imago Relationship Therapist an "out of network provider".
